The YFDA registration process collects complete information about the child, parent or guardian, emergency contact, medical requirements and preferred academy program.
After completing the registration form, the system generates an official YFDA registration contract PDF. The parent or guardian must download, print, sign and upload the signed contract together with all required supporting documents.
Before You Start
Prepare Child Information
Full names, date of birth, residential address, school, preferred position and football experience.
Prepare Parent Details
Full name, relationship, phone, email, identification information and residential address.
Review Medical Information
Medical conditions, allergies, medication, previous injuries and emergency contact details.

Registration Attachments

Signed YFDA Contract
Upload a clear PDF copy of the official registration contract signed and dated by the parent or guardian.
Child Photograph
Upload a recent and clear passport-style photograph of the child in PNG, JPG or JPEG format.
Birth Certificate
Upload a clear PDF or image copy of the child's birth certificate or another academy-approved proof of age.
Parent Identification
Upload a clear copy of a national ID, passport, driving licence, voter ID or another academy-approved identification document.
Parent and Guardian Commitments
The parent or guardian must review the contract carefully before signing and submitting the application.
Accurate Information
Confirm that all player, parent, emergency and medical information is complete and accurate.
Medical Disclosure
Declare all known medical conditions, allergies, medication and previous injuries relevant to safe participation.
Academy Rules
Accept academy rules, safeguarding standards, training requirements and disciplinary procedures.
Contact Updates
Keep parent, guardian, emergency and medical contact information current throughout the registration period.
Fee Obligations
Approved applicants must follow the academy fee schedule and payment requirements communicated after approval.
Safe Participation
Notify the academy immediately about illness, injury or any circumstance affecting the child's safe participation.
What Happens After Submission?
The academy reviews the registration form, signed contract, attachments, medical information and requested program.
The application may be marked as submitted, placed under review, approved or rejected. The parent or guardian can monitor progress using the application number and tracking code.
When an application is approved, the related player and parent records are created inside the internal management system. The player may then be assigned to an approved team and academy fee.
Submitted
The completed application has been received and is waiting for internal review.
Under Review
Academy staff are reviewing the information, signed contract and uploaded documents.
Approved or Rejected
The final decision is recorded in the system and communicated to the parent or guardian.
